Please take a few minutes and read through the following so that when you come to the clinic for your appointment your registration process will be smooth.
Services are generally provided by appointment, but Same-Day appointments are also available. Please call ahead if possible.
We do not provide urgent or emergency services. Please call 911 if you need urgent or emergency services.
For urgent health problems after hours or on weekends, please call a MayView clinic and follow the directions to speak to a triage health professional at Valley Medical Center. Or you can walk in to the Express Care Clinic at Valley Medical Center, which is open for a few hours past midnight.

Have the following information ready when you call:
- Full name of patient
- Date of birth of patient
- Insurance information
- Onset of illness/request for medical service
Your First Appointment
Please arrive for your appointment on time with your Identification (full name and address), insurance cards, and current emergency contacts and telephone numbers. If you are coming for an immunization, you will also need to bring your immunization record. Please bring in any medications you are currently taking.
If you have any questions about MayView and our practices, our staff will be happy to answer them before your exam so that your medical provider will be able to spend more time on your health issues.
We strongly encourage you to keep your appointment. If you can’t make your appointment, please call us to reschedule or cancel it in time for us to schedule another patient. Please let us know as soon as possible if you cannot make your appointment – preferably at least 24 hours in advance.
When you come to the clinic, you will be asked to fill out a health questionnaire by a staff member, and a medical assistant will take your vital signs. Your health care provider will review the health questionnaire with you, and after the examination, will design with you a treatment plan and schedule future visits, as needed. Before you leave, please be sure that you have all your questions answered and are clear about the advice and directions given by your provider.
Fees and Eligibility
MayView accepts new patients who are insured through Medi-Cal, Medi-Cal Expansion, Medicare, Santa Clara Family Health Plan and Blue Cross Health Plan. Uninsured patients are always welcome and service fees will be based on income and family size. Uninsured patients are “self-pay” and may be eligible for discounts according to our Sliding Fee Scale. There is at least a nominal charge for all services and MayView is not a free clinic.
MayView participates in Healthy Families, Healthy Kids, Cancer Detection Program (CDP), Comprehensive Perinatal Services Program (CPSP), Family Planning Access Care and Treatment (FamPACT), Primary Care Access Program (PCAP), and the GATEWAY program, which guarantees access to preventive care for uninsured children. Please ask at the Front Desk for assistance with applications for these programs.

What is PCAP? The Primary Care Access Program (PCAP) is a low cost coverage program that provides primary health care services to low-income adults living in Santa Clara County, who do not have health insurance. If you qualify and select MayView, you will be assigned MayView as your primary care provider and your “medical home.”
Who is eligible for PCAP?
- Adults between 19 years and older who live in Santa Clara County;
- Household income at 200% or less of the Federal Poverty Level; and
- Adults not covered by employer health insurance and who are not eligible for full scope Medi-Cal or Covered California subsidies.
Where can I enroll? You can enroll at MayView by calling Cuca at 650-965-3323 extension 1054 Monday through Friday 8:15am to 5pm.
What do I need to provide? You will need to provide copies of the following documents with your application:
- Proof of identity
- Proof of Santa Clara residency
- Proof of income
