Please take a few minutes and read through the following so that when you come to the clinic for your appointment your registration process will be smooth.
Services are provided by appointment only. Same-day appointments are available based on medical need. We do not provide urgent or emergency services. Please call 911 if you need urgent or emergency services.
For urgent health problems after hours or on weekends, please call a MayView clinic and follow the directions to speak to a triage health professional at Valley Medical Center. Or you can walk in to the Express Care Clinic at Valley Medical Center, which is open for a few hours past midnight.

Have the following information ready when you call:
• Full name of patient
• Date of birth of patient
• Insurance information
• Onset of illness/request for medical service
Your first appointment
Please arrive for your appointment on time with your Identification (full name and address), insurance cards, and current emergency contacts and telephone numbers. If you are coming for an immunization, you will also need to bring your immunization record. Please bring in any medications you are currently taking.
If you have any questions about the clinic and its practices, our staff will be happy to answer them before your exam so that your medical provider will be able to spend more time on your health issues.
It is very important that you keep your appointment of call us to cancel it in time for us to schedule another patient. Please inform the clinic as soon as possible if you cannot keep your appointment, preferably at least 24 hours in advance.
If you are 15 minutes late, you will be requested to reschedule your appointment. After three no shows the clinic will no longer give you an appointment. You can still come in as a walk-in patient, but we cannot guarantee that you will be seen by a medical provider.
When you come to the clinic, you will be asked to fill out a health questionnaire by a staff member, and a medical assistant will take your vital signs. Your health care provider will review the health questionnaire with you, and after the examination, will suggest a treatment plan and future visits, if necessary. Before you leave, please be sure that you have all your questions answered and are clear about the advice and directions given by your provider.
Important notes
MayView is not a free clinic. We accept the following insurance plans: Medicare, Medi-Cal, Medi-Cal Managed Care Plans (Santa Clara Valley Health Plan, Blue Cross Health Plan), VHP Valley Care and Valley Health Plan (Commercial). If you have no insurance, you will be charged based on a sliding-fee scale (based on your reported income). Co-payments and sliding-fee charges must be paid on the day of your visit.
