Collaborative Initiatives
In-Clinic Breast Health Screening
In collaboration with the Community Health Partnership of Santa Clara County, MCHC is able to offer our patients onsite mammography services. Through the CMAP project, a mammography team comes to each of our clinics once a year, a service that is especially appreciated by patients who would otherwise have to travel to Valley Medical Center in San Jose for this screening.
Quality Improvement (QI) Initiatives
MCHC participates in three QI initiative collaborations with CHP and California Primary Care Association (CPCA). PHASE (Preventing Heart Attacks and Strokes Everyday) was pioneered at Kaiser Permanente. AQIC (Accelerating Quality Improvement through Collaboration) is directed by CPCA. The iCare project includes CHP, the Community Clinics, UCSF and UCLA. All of these projects support quality improvement for patients with chronic disease, increase use of best practice guidelines, and standardize and improve measurement of quality indicators. These projects help our staff focus effectively on data collection and on provider and clinic staff training. They also help providers improve health outcomes.
