Electronic Health Records in MayView clinics
In June 2011, MayView installed a new practice management system. In December 2011, we will go live with electronic health records. In 2012, we will be able to electronically chart and maintain health information that will help our providers deliver high quality health care to all our patients, especially those whose conditions require monitoring. We hope our patients will benefit greatly from this transition, and we plan to document their health status improvements to demonstrate the level of care we provide here at MayView.
Integrating Behavioral Health in MayView’s Primary Care Clinics
In January 2012, MCHC will introduce a new program, funded by the County of Santa Clara, Mental Health Department, called Integrated Behavioral Health (BH) in a Primary Care Setting. This program, based at our Mountain View clinic, will provide integrated BH prevention and early intervention service, connecting patients through an “in-reach plan” that supports patient referrals and the use of available services.
In addition to providing BH services in our clinics, this program will establish and strengthen referral relationships for specialized services for our patients who need them. There will be a simultaneous focus on health and BH issues supporting patient self-management, which is expected to produce improvement in clinical outcomes for acute conditions through assessment, treatment, follow-up, and monitoring; improved medication adherence; and consultation and training for the primary health care team.
